Justin: Refinement Completed

Justin

Chief Complaint:

Crowding of front teeth

The photos show Justin’s completed tooth movement. You will notice in the intraoral pictures with the aligners in place, on the right side, you can see through the aligner to the healing abutment of the implant in the #29 area. The aligners fit the teeth, and the space distribution for the implant restoration is equal to the M-D width of the first bicuspid #28.

Justin: Final Aligner Delivery & Attachment Removal

Justin

Chief Complaint:

Crowding of front teeth

Justin has completed aligner wear and presents having finished wearing upper aligner #11 and lower aligner #10. As you can see in the photos, the arch shape looks good but there are several anterior rotations and some arch form symmetry to complete. At this point the results are good but need some additional movements for a “stellar” finish. The solution is to use a refinement that is included with the Invisalign Full product. The refinement provides for a new ClinCheck that we may modify to create a new series of aligners to achieve the desired results.

Justin presented for his third monitoring visit. Evaluation of the aligner fit and observation of the attachment fit appeared to be satisfactory. There did appear to be some slight movement lag in the extrusion of the upper incisors as well as slight rotation lag of the lower incisors. After some discussion with Justin he agreed that he had been less than consistent in his overall wear time.

Justin arrived for his second monitoring visit and did not report any problems. After inspection with his aligners in place the tooth movements appeared to be following the virtual representations. There appeared to be minimal movement lag with some slight incisal edge lag apparent in the case of the maxillary extrusion. Justin easily removed the aligners. Even though there was no IPR in this case, we used floss to check for tight contacts.

Justin: Surgical Guide and Implant Placement

Justin

Chief Complaint:

Crowding of front teeth

The overall treatment plan for Justin called for removal of the retained lower right deciduous second molar and placement of an implant fixture. Following implant placement, records were sent to Align. Using ClinCheck a proper M-D space was developed for the restoration of the implant fixture. This plan would allow the implant fixture to integrate while simultaneously moving teeth with Invisalign, thus saving the considerable time of implant integration before implant restoration.

Justin: Attachments

Crowding of front teeth

Let’s look at the attachment rationale for Justin.

In the maxillary arch several different attachments were placed. Following Align’s protocols, vertical rectangular attachments were placed on the maxillary left second bicuspid and right canine for the primary purpose of rotation. In this case, the rectangular attachments should also provide sufficient retention and minimize the need for additional attachments.

Let’s use Justin to summarize the delivery steps and attachment placement techniques we outlined for our other patients, Sheena and Stephanie. In Justin’s case we had to execute treatment to remove a retained deciduous tooth, create a surgical guide for implant fixture placement and place the actual implant fixture. I will outline the construction of the surgical guide and implant placement in a separate blog entry.

Justin: ClinCheck Review Process

Crowding of front teeth

Let’s review our initial treatment plan for Justin:

  • Resolve upper and lower crowding
  • Resolve canine rotations
  • Maxillary expansion and procline with no IPR
  • Mandibular expansion and procline
  • Resolve mandibular right canine crossbite
  • Reduce space of “T” for space management and to aid implant restoration
  • Maintain CL I A-P relationship
