About Dr. Clark

Dr. Clark D. Colville is an assistant clinical professor in the Orthodontic department at the University of Texas Health Science Center Houston Dental Branch. Dr. Colville graduated from UTHSC San Antonio Dental School in 1989 and received a certificate from St. Francis Hospital and Medical Center the following year after completing a hospital-based general practice residency. In 1993 he completed his graduate orthodontic training at UTHSC Houston Dental Branch and received both a certificate and a Master's degree.

Daniel has been wearing his Vivera retainer full time since April 2009. Initially, the retainer was used to recapture the exact position of the teeth at the end of case refinement. After he had lost a Hawley retainer, the initial fit of the Vivera revealed there had been some shifting of the upper teeth because the retainer did not fully seat. After wearing the Vivera for 2 weeks, the teeth were fully seating in the retainer. I added dimples using the dimple plier to add retention in the posterior areas.

Daniel: Case Refinement 8, plus 3 overcorrection

Daniel completed all 8 refinement aligners and then moved on to the overcorrection aligners in both arches (Figures 4, 9). Tooth 8 and tooth 27 needed additional rotation correction. Both teeth had 4 degrees of overcorrection planned. The contacts were checked closely to make sure space was available. As an added measure, I added dimples to enhance the rotational moments of force on these two teeth.

Daniel is wearing his case refinement aligners. I last reported Daniel’s progress when we sent new PVS impressions for case refinement.

The specific goals for case refinement are as follows:

Daniel returned today and was excited to have completed the initial series of aligners. At this point all the attachments were removed, the teeth were shaped to optimize the esthetic appearance, and the occlusion was checked closely to determine what needs to be accomplished in case refinement.

These are the final 3 aligners of the first set of aligners. Aligner 31 in the upper arch is a surgical shift aligner and will not be worn. This occurs because the surgical shift of the upper arch is read by the Treat software as a change in position of the upper arch; thus another aligner is manufactured.

A lot of treatment decisions were made today. The current aligner (23) fit well in most areas. Looking critically with the aligners in place, in the upper arch the upper right lateral was lagging slightly, less than 0.5 mm.

Daniel came into the office one week ahead of schedule and had worn aligner 19 for one week. We gave Daniel the next four aligners in the series, 20 through 23, and asked him to continue wearing aligner 19 until he had completed the second full week of wear. We are still performing IPR with a hand strip due to tight contacts. The areas in the upper arch that required reduction were the contacts between teeth 6-7, 7-8, 10-11, and 11-12.

Daniel came in today, a little ahead of his scheduled appointment. He still has one week remaining to wear aligner 15. We are now half way through the initial set of aligners.

His aligners fit well and he has not had any problems to date. The attachments were all in place and showed no signs of wear.

Daniel was seen at his regularly scheduled appointment and we delivered aligners 12-13-14-15. His current aligners are fitting well in both arches. All the contacts were checked and I lightened the contact between teeth 11-12 with a 0.10 vision flex hand strip. The cuspid (tooth #11) is rotating distally to get proper alignment, and any significant contact could prevent the rotation from occurring.

Daniel came in for a regular visit after wearing aligners 4-5-6-7. The aligners fit well and he reported that he is wearing them at least 20 hours per day. I suspect that he may not be wearing them quite that much. All attachments are in place.
